Apply for Master Pooled Asset Trust

To establish an individual trust, we need the completed application, signed/completed agreement to establish an individual trust, signed statement regarding the LA+ Master Pooled Asset Trust and a check to fund the trust. The check should be made out to Client’s Name Trust.
