How to establish a trust with LA+

If you are interested in having us manage a trust written by you or someone else:

STEP 1: Either have the person who wrote the trust, your attorney or yourself contact us by phone, (206) 367-8055 or by email, danika@laplus.org and tell us a bit about your situation:

  • Your name, age, public benefits that you receive (if any), the type of trust you are interested in, court involvement (if any), and any other relevant information.

STEP 2: Complete trust establishing documents, dependent on the type of trust you have.

  • LA+ Master Pooled Asset Special Needs Trust: Return a completed Application, Agreement to Establish an Individual Account, and Statement Regarding MPAT (all forms available here)

  • Stand-Alone Trusts
    - Provide the trust instrument to our intake team for review
    - Execute any needed documents (resignation of trustee, declination to serve, order appointing successor trustee etc.)
    - An attorney may be needed to appoint LA+ as successor trustee

STEP 3: Trust receives funding and is then established.

STEP 4: We obtain a tax identification number for the trust and open an individual trust bank checking account.

STEP 5: Our intake team begins the onboarding process. You can start utilizing your trust.

STEP 6: You will be assigned to a trust manager who will be your main point of contact for the trust and is able to deliver beneficiary-centered service.
